To order HACSG literature or take out a subscription, simply download and print out and complete the ORDER FORM (pdf) and send it to us with your payment. You can pay by cheque ( made out to The Hyperactive Children's Support Group) or by Postal Order.

If you don't have the Adobe Reader on your computer, you can download it free from the Adobe websitehere.

If you decide that you would like to join the HACSG, or that you would simply like to receive the Initial Parents Pack, or a copy of the Guide For Parents (or other literature), you can do so by printing out and completing this form and sending it to The Hyperactive Children's Support Group, Dept.W., 71 Whyke Lane, Chichester, West Sussex PO19 7PD, England or by phoning 01243 539966 between 10am and midday (weekday mornings only).
